Application of Prick by Prick test in diagnosis of food allergy in patients with seasonal allergic rhinitis – cases studies

The aim: Presenting four interesting cases seasonal allergic rhinitis and food allergy found in family (father and 3 sons).
Material and methods: Four patients treated in the Outpatients Clinic of Allergic Diseases, Department and Clinic of Allergology, Immunology and Internal Diseases, Collegium Medicum in Bydgoszcz, University of Mikolaj Kopernik in Torun. Every patient was interviewed for allergy history and family history of diseases using a questionnaire with full anonymity of patients.
Every patient had:
1. Skin-Prick test for inhalatory allergens (weed, trees, grass pollen) and chosen fruit and vegetables.
2. Skin Prick by Prick test with fresh fruit and vegetables.
3. Established the level of total and specific IgE.
Results:
Patient 1.
Male, 55 years old, 2 years of therapy. Skin Prick tests positive for weeds, no positive results for food and vegetables. Immunology: tIgE=407,26 kU/l, sIgE for weeds in II class. Prick by Prick tests positive for apple and tomato. In the interview patient complaint about symptoms after eating apple and tomato.
Patient 2.
Male, 29 years old, year of therapy. Skin Prick tests positive for birch and celeriac. Immunology: tIgE=278 kU/l, sIgE for birch in IV class, grass in II class. Apple, carrot, tomato in I class. Prick by Prick tests positive for celeriac, apple, carrot. In the interview patient complaint about symptoms after eating celeriac.
Patient 3.
Male, 25 years old, year of therapy. Skin Prick tests positive for grass and celeriac. Immunology: tIgE=1167,0 kU/l, sIgE for grass in III class. Prick by Prick tests positive for celeriac, apple, tomato, carrot, lemon. In the interview patient complaint about symptoms after eating celeriac, apple, lemon.
Patient 4.
Male, 18 years old, 10 years of therapy. Skin Prick tests positive for grass and celeriac. Immunology: tIgE=268,0 kU/l, sIgE for grass in III class. Apple, carrot, tomato, peanut – II class. Prick by Prick tests positive for celeriac, tomato, carrot, lemon. In the interview patient complaint about symptoms after eating celeriac and tomato.
Conclusions: There is a need for careful diagnosis of food allergy in patients with seasonal allergic rhinitis. The diagnosis should include Prick by Prick tests and immunology.
